We are seeking a motivated Senior Comminity IV Nurse to join our Patient Solutions team, delivering high-quality care as part of the Baxter Homecare Programme.

This is a community role where you will combine clinical expertise with leadership responsibility, supporting both patients and a small team of nurses across your region.

Job description

As a Senior Clinical Nurse, you will:

• Lead, support, and mentor a small team of nurses (up to 5)
• Deliver safe, effective, and patient-centred care in homes, hospitals, and via virtual consultations
• Support patients transitioning from hospital to community care
• Build and maintain strong relationships with patients, carers, NHS hospitals, and key stakeholders
• Monitor and manage performance against KPIs, audits, and service standards
• Deputise for the Nurse Manager when required
