Job summary
We are looking for an experienced, organised and compassionate CHC Senior Clinical Assessor to join UB Healthcare as we continue to expand our support to NHS and social care partners.
This is an excellent opportunity for a motivated clinician with a strong understanding of Continuing Healthcare, All-Age Continuing Care and the relevant National Frameworks to play a key role in delivering high-quality assessments, reviews and recommendations that support safe, person-centred and evidence-based decision-making.
Working closely with individuals, families, representatives, NHS teams, commissioners and wider health and social care professionals, you will help ensure people receive the right care, appropriately funded and aligned to their assessed needs.
The role requires strong clinical judgement, excellent communication skills and the ability to work collaboratively with system partners to meet key performance indicators, improve outcomes and maintain quality, consistency and assurance across statutory services.

Main duties of the job
Key Responsibilities
- Carry out comprehensive CHC assessments, including Checklists, Decision Support Tools (DSTs), and Fast Track assessments. As required (face-to-face and remote).
- Coordinate and lead multidisciplinary team (MDT) meetings with health and social care professionals, families, and providers.
- Produce clear, evidence-based reports and make informed recommendations on CHC eligibility. Maintain accurate case records.
- Review personalised care packages to ensure clinical effectiveness and value for money.
- Provide expert advice on CHC processes.
- Identify and manage clinical risks, safeguarding concerns, and incidents appropriately.
- Contribute to service development, audits, governance, and quality improvement initiatives.
- Liaise with social workers and other stakeholders.
- Promote excellent customer care throughout the CHC process.

Who We're Looking For
- Experienced CHC Clinician with a Strong understanding of CHC processes and the National Framework
- Registered Nurse (Adult, Mental Health, or Learning Disabilities) with valid NMC registration
- Experience in holistic clinical assessment and complex case management
- Excellent communication, report writing, and organisational skills
- Knowledge of safeguarding, Mental Capacity Act, and best-interest decision-making
- Full UK driving licence and willingness to travel
- It is Imperative that you are able to travel to meet business requirements
- Confident communicator with excellent interpersonal skills
- Skilled in Microsoft Office and digital systems used in healthcare settings
- Positive, self-motivated, and performance-oriented

An Enhanced DBS check is required for this role.

You will be expected to maintain NMC registration and work in line with the NMC Code, as well as adhere to all organisational policies, safeguarding procedures, GDPR requirements, and CQC standards. Continuous professional development and participation in supervision and appraisal are essential.
