Job summary

The Community Mental Health Nurse will work as part of one of the four Neighbourhood Mental Health Team in Tower Hamlets, providing high-quality, person-centred care to adults experiencing complex mental health needs.

The post holder will be essential to delivering care that focuses on response to crisis presentations, delivery of early intervention and proactive management that leads to theo Reduction in admissions for high-intensity service userso Reduction in ED cubicle utilisationo Improved use of inpatient capacity

They should foster Community and Connection and should be compassionate and show empathy.

The post holder should have a genuine desire to work in a multi-disciplinary and multi-agency team. They should be committed to co-production, facilitation and liaison with service users, local residents, local VCFSE, health and social care services in order that people with co-morbid physical and mental health conditions, experience good outcomes for their mental health.

They will hold a caseload and be responsible for the assessment, care planning, coordination, and review of care in line with the Care Programme Approach (CPA) and relevant clinical frameworks.

Main duties of the job

  • Develop and maintain a high standard of practice based on assessment through the use of pscyho education, the Clinical Model and supportive clinical management.
  • Liaise and establish good working relationships with colleagues in Emergency Department, Primary Care services, Social Services, Community Mental Health Teams, Assertive Outreach, In-Patient services, Voluntary Sector and other agencies as appropriate.
  • To haveknowledge of and practice a range of therapeutic interventions available to reduce distress for individuals experiencing an acute mental health crisis.
  • Ability to work autonomously and within MDT setting.
  • To work with the multi-disciplinary team in developing plans that offer supportive crisis care in the least restrictive way.
  • Role model in the practice area, encouraging staff in teaching and supervision to sustain a supportive learning environment.
  • Be responsible for the maintenance and accurate recording of patient information in the documentation systems and to ensure the effective dissemination of service user information throughout the multidisciplinary team.
  • Ensure compliance with the requirements Safeguarding Adults and Children's policies. To ensure that staff have the required knowledge and skills to work within these procedures to support the safety of vulnerable adults and children.
  • Recognize the role of advocacy, promote concept of autonomy and self-empowerment and ensure members of the team comply with this.

Ensure that you are suitably skilled in addressing the physical health needs of service users.

To represent the team positively both internally and externally to the Trust.
