Job summary
To work within the Learning Disability and Forensic (LD&F) directorate across LD inpatient services based at Little Plumstead Hospital, with the majority of their clinical caseload at the Broadland Clinic. The post holder will also work across Norfolk within the Norfolk Forensic Community LD (NFC-LD) team and Intensive Support Service (ISS). This role involves offering SLT assessment and intervention for service users with a learning disability and complex and intersecting mental illness and physical health needs. This will be undertaken in partnership with:

- Members of the multidisciplinary team
The post holder will be line managed within the Learning Disability and Forensic directorate and will be professionally accountable to the Speech and Language Therapy Professional Lead.
Main duties of the job
To manage a defined caseload, using evidence-based, person-centred principles to assess, plan, implement and evaluate assessment and intervention work packages for the specified service user group.
To work as an effective, integrated member of a multi-disciplinary team.
To establish effective communication networks with services users and carers, team members, AHP colleagues and other agencies.
To provide consultation, specialist risk assessment & risk management recommendations.
To provide highly specialist advice, support, education and training to increase the capacity and capability of others to work with people with a learning disability.
To develop Speech and Language Therapy provision within the service with support from the SLT Professional Lead.
To undertake supervision and leadership of junior staff and students as required.
To deputise for the Therapies Professional Lead in Learning Disability services as necessary
To have a key role in developing, shaping and transforming services.
Job description
Key Relationships:
To work closely with other members of the multidisciplinary team at the Broadland Clinic and across Little Plumstead Hospital; including the Broadland Clinic, Astley Court, the Intensive Support Service, and the Norfolk Forensic Community LD team.
To work with service users and carers across the aforementioned services.
To work with other stakeholders including Health Facilitation, Community Learning Disability Teams, Primary and Secondary Care Services.
To link with other SLTs, particularly those in the Learning Disabilities and Forensic business stream.
To work closely with the Professional Lead for Speech and Language Therapy.

Clinical Responsibility
To assess, diagnose and develop treatment plans for service users with learning disabilities, additional sensory and/ or physical disability, mental health diagnosis and/or challenging behaviour.
To assess, diagnose and develop treatment plans for service users presenting with dysphagia.
To coordinate and participate in videofluroscopy assessments of swallowing as required.
To have knowledge and understanding of the application of alternative and augmentative communication.
To contribute to the formulation and implementation of a multi-disciplinary recovery-based care plan within the Care Programme Approach.
To have responsibility for the planning, delivery, monitoring and evaluation of Speech and Language therapy assessments and interventions.
To be responsible for managing a defined caseload, in the context of the service caseload, with ongoing evaluation of clinical effectiveness using specified outcome measures, audit and service evaluation.
To work with and be available to other professionals for support and joint consultation regarding clinical practice as required.
To ensure that clients are consulted and involved in decision about their care
Leadership and Staff Management Responsibility
To review and reflect on you own practice and performance through effective use of professional and operational supervision and appraisal.
To support junior staff and students to review and reflect on their practice and performance through effective delivery of supervision and appraisal.
To support junior staff to develop role-based competencies and to manage capability where required.
To exercise good personal time management, punctuality and consistent reliable attendance.
To co-ordinate day-to-day activities of junior staff where applicable, providing clinical supervision and support.
To be responsible for the supervision and written assessment of SLT students on practice placement within HPFT.
To demonstrate leadership skills through the management of designated projects.
To promote awareness of the role of SLT within clinical and partner services, negotiating priorities where appropriate.
To participate in the induction, training and education of students and other staff in this setting as appropriate.
To represent the service locally and nationally where needed.
To apply knowledge, experience and skills to make decisions in the absence of senior staff.
To recruit and appoint staff in conjunction with relevant Team Leaders and Professional Leads and in accordance with the required operation of the business unit
To identify training and development needs of individual staff and teams.
To ensure Attendance Management Policies are implemented and to pass relevant information to relevant departments within the Trust.
Financial Responsibility
- To be responsible for maintaining stock, advising on resources to carry out the job.
- To be responsible for the safekeeping and use of equipment and materials.
- To support the Speech and Language Therapy Professional Lead with coordinating the Training Needs Agreement budget.
- To ensure Health and Safety Regulations and Data Protection Act Regulations are observed and instituted.
Service Development and Improvement
- To participate in the planning, evaluation and audit of practice, clinical pathways and protocols within your area.
- To support, engage in, and deliver service improvement and development projects, which may come in the form of audit, service evaluation, CQI, and research.
- To contribute to the formulation of standards and supporting the integration of new evidence-based practices within the service.
- To contribute to the Trusts Practice Governance arrangements.
- To challenge practice, in the best interest of service users, carers and the Service.
Communications
- To establish and maintain robust networks of communication with HPFT and partner agencies.
- To liaise with Team Managers and Senior Clinicians to advise on speech and language therapy clinical, professional, operational and safeguarding issues.
- To provide interpretation and dissemination of legislation, policies and procedures to staff in relation to their operational impact.
Analytical and Judgemental Skills
To demonstrate commitment to ongoing personal development and acquisition of further skills and knowledge relating to clinical areas as specified and the application of these skills to clinical practice, and through participation in internal and external development opportunities, recording learning outcomes in a portfolio.

Planning and Organisational Skills
To plan own diary and appointments with service users and carers, in conjunction with HPFT LD services based in Norfolk.
To assist in the identification of training needs of junior staff including that needed in response to new trends in practice.
Information Resources
To ensure that up to date written and electronic records, reports, and outcome measures are maintained in accordance with Professional and Trust standards.
To record appropriate statistical activity.
