Job summary

An exciting opportunity has arisen for a part time Occupational Therapist within the urgent care response and intermediate care service in Torbay. This opportunity allows you to undertake holistic needs-led assessments in partnership with service users, patients and carers.

You will support the management of a team caseload of complex, highly complex and statutory work, and act as an autonomous practitioner evidencing high levels of problem solving, decision making and independent judgement in a variety of community settings and situations.

We work in an integrated way with allied health care professionals within the locality, and are a motivated and caring team to both patients and colleagues therefore, your role with us will involve Community therapy and Intermediate care work. There is a division between these areas of work,.

To effectively undertake the role, you will be required to work flexibly to ensure a consistent and robust service, which covers seven day working including weekend and bank holiday work, or working in other localities across the bay as necessary to meet the needs of the service.

To explore innovative ways of collaborative working between general practice, health and social services and the private and voluntary sector in line with the Care Trust's integration agenda.

The post is currently 22.5 hours however there maybe opportunity to increase these hours in the future

Main duties of the job

  • Responsibility for managing an intensive actue caseload.
  • Triage and prioritise work according to urgency and establishing presenting risks.
  • Assessment and treatment of patients in a variety of community settings, which is mainly the patient's own home, but could incluse nursing homes and residential homes.
  • Working with a patient centred approach, taking responsibility for planning , implementing and reviewing OT interventions for patients and making autonomous decisions/recommendations regarding packages of care, equipment provision and ongoing treatment to maintain patients at home, thereby preventing hospital admission. Interventions include manual handling assessment and equipment provision, simple and complex equipment and adaptation prescription.
  • To carry out home assessment visits with patients from temporary places of safety, thereby contributing to the planning and facilitation of discharge to their own homes as appropriate.
  • Supervisory responsibility for a Band 5 post , Assistant Practitioner or support worker as required.

Job description

  • Diploma/Degree in Occupational Therapy
  • Valid state registration with Health Professions Council
  • Proven post-graduate experience
  • Evidence of ability to make decisions under pressure and in stressful situations
  • Moving and handling protocols and procedures to include provision of hoists and specialist slings
  • Experience of supervision of junior staff and/or OT students
  • Highly developed communication skills to include negotiation andexperience of communicating sensitive information e.g. unfavourable assessment outcome
