Job summary
We have an exciting job opportunity for a Practice Nurse to join our nursing team.
Applicants should possess appropriate nursing qualifications and be able to work both as part of a team and independently.
Knowledge of COPD/Asthma is desirable but not essential.

Main duties of the job
To care for Practice patients, including the provision of treatment, preventative care, screening, immunisations and patient education. At all times demonstrating a working knowledge of the conduct and standards required of a Nurse, as required by the Nursing and Midwifery Council.
Providing assessment, screening and treatment services and health education advice.
Pathological specimens and investigatory procedures.

Practice Nurse Responsibilities
- Provide cervical screening (smear tests) and sexual health advice.
- Administer childhood and adult immunisations, travel vaccinations, and seasonal flu jabs.
- Conduct wound care, dressings, suture removal, and other treatment room procedures.
- Perform ECGs, venepuncture, and NHS Health Checks.
- Provide contraception advice and support as trained.
- Manage and deliver nursing care for patients with long-term conditions such as diabetes, asthma, hypertension, and COPD.
- Contribute to infection control and cold chain compliance.
- Deliver culturally sensitive lifestyle advice on smoking cessation, weight management, sexual health, and alcohol/substance misuse.
- Promote self-management of chronic conditions and provide patient education tailored to the needs of the local population.
- Participate in Quality and Outcomes Framework (QOF) monitoring and data entry.
- Maintain accurate, up-to-date clinical records using EMIS
- Support clinical audits, safeguarding processes, and care planning.
- Attend and contribute to practice and multidisciplinary team meetings.
Professional Requirements
- Registered Nurse (Adult) with current NMC registration
- Experience working in primary care or a similar community-based setting
- Confident in managing chronic diseases and delivering immunisations and screening
- Understanding of the health challenges faced in inner-city or deprived communities
- Cervical cytology training
