Main duties of the job

HCRG Care Group is seeking a Care Coordinator (Administrator) to work within the Care Coordination Hub servicing families and young people throughout Lancashire. The role offers the opportunity to be part of a supportive team that assists children, young people, and families in accessing the necessary care in a timely manner. The successful candidate will be responsible for being the first point of contact, providing a courteous and professional response via telephone and email, and directing queries to the appropriate teams. Additionally, the candidate will support the clinical teams by scheduling routine appointments according to service protocols, ensuring timely signposting of referrals and queries, and collaborating with colleagues across the service for effective care coordination. Key skills required include excellent communication, customer service proficiency, strong organizational ability, attention to detail, and comfort with IT systems. While experience in healthcare or coordination roles is a plus, the emphasis is on people skills and a proactive attitude.

What youll do

What we're looking for

You will thrive in this role if you:

  • Have excellent communication and customer service skills.
  • Can remain calm and supportive when managing a high volume of calls or complex queries.
  • Are highly organised with strong attention to detail.
  • Enjoy working in a busy, fastpaced team environment.
  • Are confident using IT systems and eager to learn new processes.
  • Are willing and able to travel across Lancashire to support other hubs if and when required (Valid UK Driving Licence essential).

Experience in healthcare or coordination roles is desirable but not essential your people skills and proactive approach matter most.

This role is based between Preston and Burnley so ability and willingness to travel across both hubs is essential.
