Job summary

<p>Were looking for a caring, compassionate and dedicated <strong>Community Support Worker</strong> to join our team.</p><p>The role of the community support worker is to work within a multi-professional team in a community setting, providing, support, care, and treatment to individuals, within guidelines and protocols identified for the role. </p><p>The community support worker will provide support with guidance from nurse and/or therapist to deliver care plans to patients in a community setting. <strong>Depending on experience and skill mix, you will carry out nursing or therapy related activity. </strong></p><p>As our services transform, there may be times you will be expected to support both nursing and therapy in the delivery of care, within an appropriate scope or practice and following relevant on-the-job training.</p><p><strong>This is a full-time role working 37.5 hours per week.</strong></p><p><strong>Base:</strong> Melksham Community Hospital, SN12 7NZ</p><p>For any enquiries, please contact <a href="mailto:[email protected]">[email protected]</a></p><p>Main Responsibilities:</p><ul><li>Deliver treatment/care within a community setting as specified in a written care plan, under the guidance of a registered practitioner.</li><li>To keep the
