Job summary

This is a Band 5/6 post depending on experience.

We are looking for an experienced, forward thinking Physiotherapists to join our Hospital at home therapy team here in Herefordshire.

You will be working as part of an experienced, multi-disciplinary and multi-agency team alongside other Physiotherapists, Occupational Therapists, and Therapy Support Workers.

This is a countywide service therefore holding a full driving license and access to a vehicle is essential.

This post is 22.5 hours - fixed term cover for maternity leave

Main duties of the job

As an integral part of the team the post holder will assess, plan, implement and evaluate the delivery of physiotherapy in the team. You will be part of the therapy resource providing an urgent care response, as well as supporting discharge from A&E. Providing assessment, support planning and interventions to meet the person's needs in the community ensuring the delivery of high-quality care.

As a service we are innovative, forward thinking and encourage staff engagement. You will have the opportunity to be involved in service / quality improvement and develop leadership skills as well as supervising rotational Physiotherapist, Therapy assistants and engage in student placement education.
