Job summary
Waterside Health Network Primary Care Network (PCN) is looking to recruit a PCN Manager to support the member GP practices in administering the PCN contract.
The PCN Manager is a key leadership role, responsible for supporting the delivery of PCN objectives, contractual requirements, coordinating services, project management, managing PCN workforce allocation, and improving patient access across. They will work independently and support the Clinical Directors and practice management teams in the execution of the PCN Direct Enhance Service contract.

Main duties of the job
The PCN is looking for someone with a good knowledge and understanding of Primary Care who can manage the delivery of a number of projects across all of the PCN practices. You will be self-motivated with a pragmatic can-do approach. You will work independently within a supported environment. The role is pivotal in leading the PCN to improve the quality of care and operational efficiencies, requires motivation and passion to deliver an excellent service within general practice. The role will predominantly provide primary support to our GP practices in the delivery of the PCN DES Contract.
1. Strategic Leadership & PCN Business Planning: Leading development, delivery of a PCN strategy. Translate DES requirements into clear operational plans with measurable outcomes. Lead PCN related workstreams.
2. Programme & Transformation Management:Lead planning, implementation and evaluation of transformation programmes including personalised care, digital and data initiatives, workforce development and neighbourhood level projects. Develop programme documentation, risk logs, delivery plans and progress reports.
3. Workforce Leadership:Provide overview of PCN ARRS staff. Support ARRS staff to work consistently across practices, embedding standardised processes and shared operating models.

Job description
The Primary Care Network (PCN) Manager is a lead role within a complex environment of primary care management. They will deliver on a wide range of business, data submission, transformation and improvement initiatives across the PCN primary care locality providing strategic transformation support and leadership.
The PCN manager will be instrumental in progressing its PCN development plan. This role will work with member practices to break down challenges to implementation across our population.
The PCN manager will develop strong relationships and engage with a wide range of stakeholders across the locality to ensure collaborative approaches to service development and delivery. In doing so they will improve integration with the broader community services as our neighborhood develops.
The PCN manager will be responsible for managing agreed projects with a focus upon developing the partnerships across primary care, community services, social care, mental health, the voluntary sector and secondary care that are enabling on-going Multi Disciplinary Team (MDT) development.
The PCN manager will be responsible for disseminating new information across the member practices and tasked with coordinating shared learning events. They will actively support integrated neighbourhood ways of working whilst promoting standardisation for efficient purposes.
This role will be outward looking to help secure our resilience by actively seeking development opportunities and by coordinating proposals.
The PCN manager will be essential in supporting recruitment to PCN roles under the additional roles reimbursement scheme and integrating new roles into our primary care teams and supporting new staff.
The following are the core responsibilities of the PCN manager. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
a. To work as part of a multi-disciplinary team and to provide day-to-day managerial supervision to the PCN by leading and supporting quality improvement and performance
b. To support recruitment, induction and training of new PCN staff and to ensure that all have the appropriate qualifications, registrations, training, accreditation and requisite skills to undertake the duties required of them at each practice
c. To work closely with the relevant teams and practices to support network implementation and standardisation of agreed service change, contracts, policies, pathways and quality improvement whilst acting as the point of contact for PMs with regard to delivery of a PCN project/scheme
d. To liaise and work with different practices on an ad-hoc project basis
e. To be accountable for all projects and to ensure patient safety and quality of care is always considered and establish necessary monitoring and tracking processes
f. To help staff within the PCN to understand and support the delivery of its strategic objectives and outcomes framework
g. To collate feedback/analysis data on behalf of the PCN to report to the ICB
h. To co-ordinate the delivery of enhanced services and other service submissions on behalf of the PCN
i. To provide management oversight at board/strategic meetings
j. To develop and run administrative systems for the PCN collaboration projects
k. To work alongside PMs within the PCN for the setting up and maintaining of new financial systems
l. To monitor and check the accuracy of the any submissions for enhanced services and contractual requirements on behalf of the PCN and to ensure that all claims are submitted in good time and payment made and received
m. To ensure financial risks are recognised and appropriate action taken
n. To monitor expenditure, plan and monitor income and cash flow to ensure that income is maximised and that funds are available as required and practice allocation of resources if fair and equitable
o. To present financial plans, strategic goals and any monitoring information and reports to the PCN on a regular basis
p. To ensure the PCNs compliance with GDPR and FOI along with other relevant legislation and professional standards from GMC/BMA and LMC guidance
q. To review any latest guidance and to liaise with both PCN staff and PM to ensure that all are aware of and conform to NHS E, NICE, CQC requirements
r. To manage risks and issues with supporting outcomes. These are to be reported to the PCN clinical director
s. To assist in the resolution of any complaints, resolving informal complaints locally and assisting in the investigation of and response to formal complaints as required
t. To work with the PCN clinical director for strategic and operational planning processes to support the PCNs objectives
u. To support delivery of IIF, incentive schemes, and other quality or cost effectiveness initiatives
v. To maintain a business development plan
w. To maintain a business continuity plan
x. To develop yourself and the role through participation in training and service redesign activities.
y. To undertake all mandatory training and induction programmes
z. To contribute to and embrace the spectrum of clinical governance

Waterside Health Network Primary Care Network (PCN) consists of 7 GP practices providing GP Primary Care services to 71,000 patient population. The operating model of the PCN preserves practice-level independence. Patients registered within the network remain with their individual GP surgeries. They continue to access day-to-day general medical consultations, physical examinations, and routine prescriptions through their registered practices. The network operates behind the scenes supporting by conducting improvement projects, weekend and evening clinics, specialised chronic disease management programs and working with system partners.
