Job summary

37.5 Hours, Fixed Term for 12 months

An exciting opportunity has arisen for an enthusiastic person to join the Intermediate Care rehabilitation team.

The successful candidate will work as part of a team to aid enablement and provide rehabilitation to patients across the Rotherham Borough. The team includes physiotherapists, occupational therapists, specialist mental health occupational therapists, speech and language therapists and therapy support workers.

The team works closely with a wider multidisciplinary team (MDT) of social workers, nurses, doctors, Unit managers, care and enabling staff and voluntary sector.

The successful candidate will hold their own caseload as well as supporting qualified staff to provide therapy for their patients throughout their time within intermediate care. The post holder will be provided with support to develop and build on competencies within the clinical setting.

The successful candidate needs to be able to work autonomously and as part of the MDT. They need to have good communication skills both verbally and non-verbally. They need to be comfortable performing personal care tasks for patients and comfortable working with patients who have complex physical, emotional and psychological challenges.

Main duties of the job

The successful candidate will require a flexible approach. Excellent team working, organisational and time management skills are essential.

To undertake initial clinical assessment of rehabilitation patients.

To progress and modify treatments up to level of discharge, following training and guidance from qualified physiotherapy and occupational therapy staff.

Directly provide treatment/ rehabilitation to a patient or group of patients.

Assessment for and delivery of treatment/rehabilitation packages of care

Assess the environment and educate the patient as necessary as part of assessment. To include assessment of a patients own home.

Supervision and assessment of patient/s undertaking a therapy/rehabilitation programme, including activities of daily living

Assisting in the personal care needs of patients as required

Identify and assess equipment suitability and provide instruction.

To be responsible for equipment used in carrying out duties, adhere to Trust and departmental policy and ensure the safe use of equipment by others through teaching and training.

To be responsible for maintaining own competency and self-development through Active involvement in the PDR process.

Maintaining awareness and up to date knowledge as appropriate for specialist area.

Maintenance and development of clinical skills and apply knowledge of evidence-based practice within designated areas.

To support with clinical audit and service development where appropriate
