Farnham Primary Care Network is looking for a Pharmacy Care Co-ordinator to join our friendly and supportive Pharmacy Team. This is an exciting opportunity to play a key role in helping patients receive safe, effective and timely medicines support while working alongside Clinical Pharmacists, Pharmacy Technicians, GPs and other healthcare professionals.

Main duties of the job

What You'll Be Doing

As a Pharmacy Care Co-ordinator, you'll provide administrative and patient support to the Pharmacy Team and help deliver a range of medicines optimisation services across the PCN.

Job description

Your responsibilities will include:

• Supporting Clinical Pharmacists and Pharmacy Technicians with medicines optimisation projects.
• Contacting patients to arrange blood tests, medication reviews and follow-up appointments.
• Liaising with community pharmacies, hospital teams and GP practices to resolve medication-related queries.
• Updating and maintaining accurate patient records.
• Supporting medicines reconciliation following hospital discharge.
• Assisting with prescribing initiatives, audits and service improvement projects.
• Helping to improve repeat prescribing processes and patient safety systems.
• Supporting delivery of Quality and Outcomes Framework (QOF) targets and other enhanced services.
• Communicating medicines-related information to practice staff and healthcare professionals.
• Attending team meetings and contributing ideas to improve patient care and service delivery.
• Promoting the safe, effective and cost-efficient use of medicines.
