Job summary

We are excited to offer an opportunity to join the Integrated Community Stroke Service, working as an Occupational Therapist / Physiotherapist delivering specialist stroke rehabilitation. The role involves completing comprehensive assessments, delivering evidence-based interventions and supporting patients to work towards individualised rehabilitation goals, primarily within their own homes or place of residence.You will play a key role in supporting timely and safe discharges from hospital, identifying and managing stroke-related complications early, and helping to reduce avoidable readmissions. The post also involves providing education and practical support to patients, families and carers, promoting self-management following stroke and offering advice and signposting to other appropriate services as needed.You will support patients within a defined geographical area, however, on occasion there may be a requirement to provide countywide cover in line with service needs.

Main duties of the job

Service Aims

  • Support safe discharges from hospital and provide a high standard rehabilitation service to patients referred to the Integrated Community Stroke Service
  • Provide specialist assessment, effective treatment, and individualised goal setting of patients in their own home/ place of residence, to promote recovery post stroke.
  • Early identification and management of stroke related complications.
  • Reduce readmissions due to complications post stroke.
  • Provide education and support to formal and informal carers.
  • Promote the development of self management skills following stroke.
  • Provide advice and signposting to appropriate services
