Job summary

The opportunity has arisen to be part of our expanding multi-disciplinary frailty team and deliver care in our Proactive Frailty Care Planning Clinics across the six Integrated Neighbour Team locations in Buckinghamshire.

In our Proactive Frailty Care Planning clinics, we conduct Comprehensive Geriatric Assessments (CGAs) on moderately frail patients to identify and solve problems before things go wrong.

The postholder will assist patients and clinicians within the multip-disciplinary team to enable assessments to take place effectively to provide the necessary solutions to ensure patients can lead healthier, more independent lives.

Main duties of the job

The Healthcare Assistant will support patients within agreed protocols and clinical guidelines, working within their own competence as part of a multi-disciplinary team assisting clinicians to undertake frailty health assessments.

  • To assist the registered clinicians in the delivery of high quality and person-cantered care.
  • To provide fundamental nursing care, e.g., assistance with personal hygiene, eating and drinking, mobility needs, washing under direct or indirect supervision.
  • To carry out limited clinical observations and undertake a range of activities under direct or indirect supervision by a registered nurse, having been assessed as deemed competent in the activities required.
  • To maintain high clinical standards
  • To help maintain a clean and safe ward environment.
  • To contributes towards the department and organisation in the provision of a safe and caring environment.
  • The post holder may be required to work anywhere within the Trust.

Key working relationships include all members of the multi-professional team, including nursing and medical staff, allied health professionals and relevant support workers.
