Job summary

The Advanced Clinical Practitioner will make autonomous decisions in the assessment, diagnosis and treatment of patients registered with our practices. You will have experience within a primary care setting and your high standards of patient care will naturally inspire those you work with.

Main duties of the job

As a clinical leader within our multidisciplinary team (working alongside GPs, Practice Nurses, Care Coordinators and Pharmacists), you will help transform and modernise care pathways.

Your day-to-day will involve:

• Autonomous Clinical Practice: Making independent decisions in the assessment, diagnosis, treatment, and management of patients across full episodes of care.
• Minor illness & long-term conditions.
• Clinical Governance & Leadership: Accountable alongside the Lead Nurse for driving clinical governance, ensuring safe, compliant, and high-quality service delivery.
• Team Mentorship: Inspiring high standards of care, supporting skill-sharing across traditional professional boundaries, and mentoring team members.

The surgery has served the local community since 1874 from its Grade II listed building and is home to a dedicated team, providing care to approximately 10,000 registered patients. We also have exciting plans to deliver a state-of-the-art redevelopment of the surgery in the very near future.
