<p><span style="text-decoration: underline;"><strong>Main duties of the job</strong></span></p>

<ul>

<li>Deliver diabetes reviews and ongoing management.</li>

<li>Provide patient education and lifestyle advice.</li>

<li>Monitor and optimise diabetes treatment in line with national guidelines.</li>

<li>Undertake chronic disease management clinics.</li>

<li>Administer vaccines and immunisations.</li>

<li>Perform cervical screening, wound care and health promotion activities as appropriate.</li>

<li>Maintain accurate clinical records and contribute to quality improvement initiatives.</li>

</ul>

<p>If you&#8217;re an enthusiastic Practice Nurse who ideally has a Diploma in Diabetes, or previous experience in the ongoing management of diabetes care and are looking to join a friendly, supportive practice where your skills are recognised and your wellbeing matters, we&#8217;d love to hear from you, informal visits or an informal conversation with the Practice Manager or Practice Nurse Lead are warmly welcomed.
