We are looking for an enthusiastic and experienced Practice Nurse to join our friendly and supportive team at Moordown Medical Centre.

Moordown Medical Centre is a well-established, organised and forward-thinking GP practice caring for approximately 9,900 patients in Bournemouth. We are proud of our strong team culture and our commitment to delivering high-quality, patient-centred care.

We are ideally looking for a nurse with previous experience working within General Practice or Primary Care. Experience in any areas of chronic disease management, preferably Diabetes would be desirable but is not essential, and we would be very happy to support a nurse who is keen to develop their knowledge and skills in this area.

Main Duties

Depending on experience and competency, the successful applicant’s responsibilities may include:

• Supporting the management and review of patients with long-term conditions
• Undertaking patient reviews and health assessments
• Supporting patients with self-management and healthy lifestyle choices
• Managing patient recalls, follow-up and annual review programmes
• Monitoring and interpreting clinical results within their level of competence
• Working collaboratively with GPs and the wider clinical team
• Maintaining accurate and comprehensive clinical records
• Contributing to quality improvement and preventative healthcare initiatives

General Practice Nursing Duties

The role will also include:

• Phlebotomy
• Vaccinations and immunisations
• Cervical cytology, where appropriately trained
• Wound care and dressings
• ECGs
• Health promotion and preventative care

The exact responsibilities can be tailored to the successful applicant’s existing skills and experience, with opportunities to develop further competencies within the role.

About You

We are looking for a nurse who:

• Holds current NMC registration
• Ideally has previous experience working within General Practice or Primary Care
• Has experience in chronic disease management, or an interest and willingness to develop in this area
• Has excellent communication and organisational skills
• Can work effectively both independently and as part of a team
• Is committed to delivering high-quality, patient-centred care
• Is enthusiastic about developing their skills within General Practice

Hours

Full-time, Mon-Fri,  37.5 hours per week, although hours are negotiable for the right candidate.
