Help turn digital ideas into changes that primary care teams can use. As Programme Lead Digital Transformation, you will support projects from initial use case through rollout, training and evaluation, keeping delivery organised and helping colleagues adopt new ways of working.

This permanent, full-time, remote role brings together project delivery, operational improvement and practical implementation support. You will work with clinical and operational colleagues, digital specialists and suppliers on projects that improve access, efficiency, staff experience and patient services.

The portfolio includes automation, AI-enabled workflow redesign and other digital solutions. We welcome relevant experience in healthcare, digital transformation, operational improvement or a similar regulated environment.

Salary: From £45,000, dependent on experience. Contract type: Permanent. Hours: Full-time. Working arrangements: Remote, with national travel and occasional evening work. Closing date: Friday 30 October 2026.

Main duties of the job

This is a practical programme delivery position with the chance to see ideas become part of everyday services. You will coordinate projects and build the relationships, evidence and implementation support needed to make change stick. Reporting to the Chief Operating Officer and accountable to the Chief Clinical Information Officer, your responsibilities will include:

  • Planning and coordinating digital projects, with clear milestones, ownership, risks, dependencies and progress reporting.
  • Preparing meeting papers, decision logs, actions and updates for digital transformation forums and senior leaders.
  • Working with clinical, operational and supplier teams to capture requirements, resolve implementation issues and support rollout.
  • Developing use cases and improvement proposals for digital solutions, including automation and AI-enabled workflows.
  • Creating user guidance, training resources and practical support to help teams adopt new systems and processes.
  • Tracking adoption, outcomes, feedback and benefits, and using the findings to improve future delivery.
  • Preparing case studies, impact reports and evidence to communicate the results of digital transformation projects.

You will work across multiple teams and priorities, combining clear project discipline with hands-on problem-solving. The impact of your work will be seen not just in completed plans but in solutions that teams can confidently use and improvements that can be measured.
