We have a new exciting role of Lead Practice Nurse who will be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. An interest in Diabetes is desirable and if not trained training will be given.

This role will be based on Band 7 pay scale, although we do not work to agenda for change this is for reference only. We are looking for someone to work 35 hours per week , days to be negotiated but ideally 4 to 5 days would be desirable. there will be the need to work some saturday mornings to cover specialised clinics. This is paid as overtime.

Main duties of the job

The Lead Practice Nurse is responsible for the clinical leadership, management, and strategic direction of the practice nursing team, including Practice Nurses, Health Care Assistants (HCAs), and phlebotomists. The post holder will balance high-quality, direct patient care with operational leadership, driving quality improvement, local clinical governance, and compliance.

Core Responsibilities

1. Clinical Leadership & Team Management

  • Line Management: Supervise, mentor, and line manage the nursing team, managing rotas, annual leave, and performance reviews.
  • Appraisals & CPD: Conduct annual appraisals and support staff with continuous professional development (CPD) and NMC revalidation requirements.
  • Recruitment & Induction: Lead the recruitment, selection, onboarding, and training of new nursing staff.
  • Team Meetings: Organise and chair regular nursing team meetings to discuss clinical updates, guidelines, and operational changes.

2. Advanced Clinical Practice & Diabetes Focus

  • Diabetes Service : Part of the team that oversee the practice's diabetes management services, managing a caseload of patients with Type 1 and Type 2 diabetes.
  • Chronic Disease Management: Oversee the evidence-based clinics for long-term conditions (including Asthma, COPD, Hypertension), developing and implementing individualised care plans, desirable trained not essential.
  • Preventative & Routine Care: Oversee and deliver childhood and adult immunisation programs, cervical cytology, travel health advice, and wound care management.
  • Prescribing & Medication: Utilise independent or non-medical prescribing skills (if qualified) or administer care under Patient Group Directions (PGDs) in line with legislative frameworks.
  • Complex Patient Care: Manage complex health needs, including frail or housebound patients, coordinating integrated care plans.

3. Clinical Governance, Quality & Compliance

  • EMIS Systems Management: Ensure the nursing team maintains impeccable data quality and accurate clinical coding within EMIS Web. Utilise EMIS to run clinical reports, patient recalls, and monitor QOF targets.
  • Infection Control: Act as the Infection Prevention and Control (IPC) Lead for the practice, producing annual IPC statements, conducting audits, and ensuring practice-wide compliance.
  • Audits & Quality Improvement: Lead and participate in regular clinical audits (focusing on diabetes outcomes and QOF indicators), Significant Event Analyses (SEAs), and risk assessments to elevate standards of care.
  • Guideline Implementation: Ensure nursing protocols strictly align with NICE guidelines, Care Quality Commission (CQC) standards, and national service frameworks.
  • Policy Development: Collaborate with the Practice Manager and Partners to draft, implement, and review clinical policies and standard operating procedures (SOPs).

East trees Health Centre is based in inner city Bristol. We are a training practice with a list size of 15000 patients. We serve a very multi-cultural community

Our practice team comprises of 4 partners, 7 salaried GPs, a paramedic, nurse practitioner, physio, podiatrist,dietitians, care co-ordinator, care navigators, administrators, nurses and pharmacy team. We are a very friendly, close knit team and have regular staff and team meetings to ensure communication is consistent.
