Job summary
We are seeking a motivated and compassionate Practice Nurse to join our friendly and supportive practice. The successful candidate will provide high-quality nursing care to patients, including chronic disease management, health promotion, cervical screening, immunisations, wound care, and routine nursing treatments.
Working collaboratively with GPs and the wider multidisciplinary team, you will play a key role in delivering safe, patient-centred care and improving health outcomes within our practice. Main duties of the job

The role involves supporting patients with long-term conditions, providing health education and advice, undertaking clinical assessments, maintaining accurate patient records, and working collaboratively with GPs and the wider multidisciplinary team to ensure safe, effective, and patient-centred care.

You will work with doctors and other health workers to support patient health. A key focus of this role involves supporting patients living with long-term conditions.
