We have an exciting opportunity for an experienced and motivated Senior Medicines Management Pharmacy Technician (MMPT) to join our expanding team within one of the UK's leading NHS Trusts. Based primarily at Charing Cross Hospital, with cross-site working at Hammersmith and St Mary's Hospitals, you will play an integral role in delivering clinical pharmacy services across medical wards.

As part of our forward-thinking and supportive pharmacy department, you will work within a multidisciplinary team alongside pharmacists, doctors, nurses, occupational therapists, and other allied health professionals. Together, you will help promote the safe, effective, and efficient use of medicines, with a strong emphasis on patient education, medicines adherence, and discharge planning.Key responsibilities include:

  • Supporting medicines reconciliation and clinical screening processes on admission.
  • Conducting patient consultations and adherence assessments.
  • Educating patients and staff on medicines optimisation and safe practice.
  • Acting as a role model and mentor to junior staff, trainee technicians, and trainee pharmacists.
  • Participating in audit and quality improvement initiative.

Main duties of the job

  • Assisting in the delivery of a high-quality, patient-focused medicines management service, supporting the clinical pharmacy team to ensure safe and effective medication use across the wards.
  • Leading and coordinating the safe and efficient supply of medicines, including stock management, use of patients' own drugs, individual patient dispensing, and discharge medication planning.
  • Supporting the clinical discharge process, ensuring accurate and timely provision of medication, counselling patients on their prescribed treatments, and promoting adherence.
  • Playing a key role in educating and empowering patients about their medication to support understanding, safety, and compliance.
  • Contributing to service improvements in line with local and national NHS requirements, working closely with the Senior Lead Technician and Pharmacist to support development and implementation of ward-based initiatives.
  • Collaborating with doctors, nurses, occupational therapists, and other allied health professionals as a respected member of the multidisciplinary team.
  • Delivering education and training sessions on medicines management to other healthcare professionals, supporting a culture of continuous learning and improvement.
