An excellent opportunity has arisen for a Mental Health Practitioner in the Cheltenham, Tewksbury and North Cotswolds Assertive Outreach team.

We are looking for an enthusiastic and motivated individual to join our multi-disciplinary team. Applicants should have an interest in working with individuals who have severe and enduring mental health problems and are difficult to engage. The specific purpose is to deliver care in a collaborative manner that reflects the principles of AOT working, particularly aspects relating to recovery and social inclusion.

The Cheltenham, Tewksbury and North Cotswolds Assertive Outreach Team provides comprehensive mental health and psychosocial support to people with severe and enduring mental illnesses such as psychosis and mood disorders. The successful candidate will care co-ordinate a caseload, whilst working with the MDT and external agencies in order to promote patients recovery and quality of life. This post is open for all registered clinicians.

Main duties of the job

This is an exciting opportunity for community mental health Practitioner who has some experience of working with adults of Working Age and caring for individuals with enduring mental health problems. The post will include care coordinating a designated caseload, working within CPA Policy and associated countywide risk procedures, travelling independently, use of IT skills and a working knowledge of legislation regarding safeguarding vulnerable adults, the MHA and MCA.

Job description

  • Engage in complex health assessments as appropriate and plan relevant care in partnership with service users.
  • To act as care co-ordinator in line with Gloucestershire Health and Care Trust policy for Care Programme Approach
  • To ensure appropriate safety assessments are completed
  • To develop and deliver evidence based packages of care including specific bio-psychosocial interventions where appropriate
  • To work in partnership with service users, carers, other team members and other statutory and non-statutory agencies
  • To liaise closely with the CRHTT during periods of crisis for the service user and to ensure their timely involvement in helping to manage the crisis
  • To provide clinical supervision, caseload management, preceptorship, mentorship and day to day supervision/management for a designated staff group and students, ensuring appropriate professional development and quality of care
  • Positively address issues of discrimination and social inclusion
  • Patriciate, collaborate, educate and offer consultancy to other partner agencies and others ensuring appropriate decisions are made regarding service user and carer needs
  • To ensure that all relevant data is entered onto Trust information systems (e.g. RiO, PAS, ERIC) in a timely manner
  • Ensure the physical health needs of service users are met, including advice, support and health education to both service users and carers
  • Taking responsibility in a generic mental health team setting for tasks often traditionally associated with other professionals when safe to do so and sufficiently trained
  • To ensure all safeguarding and Mental Capacity Act issues are dealt with according to policy
  • Contribute to service development and quality initiatives in line with local and national policies through the use of audit and policy development
  • To act up for the team manager as required (band 6)
  • Undertaking other duties appropriate to the post/profession as requested by Senior Managers
