Job summary

We are looking for an experienced Lead Clinical Pharmacist for a fixed term of 12 months while our current lead is on maternity leave.

Main duties of the job

The Lead Pharmacist plays a key role in delivering and developing medicines management for the Practice. You will manage the pharmacy team, provide expert clinical support to our GPs, nurses and wider multidisciplinary team and provide support to patients with medicines queries, treatment options and advice.

The pharmacy team consists of a pharmacy technician, pre-registration pharmacist and pharmacy students, you will need experience of supervising and supporting these roles.

The Practice is part of the Deep End Network delivering care to the most deprived in the area, therefore experience in managing medicines and prescribing for this population is essential.

Job description

As the Lead Clinical Pharmacist you will:

  • Provide clinical leadership on medicines optimisation and prescribing within the Practice
  • Support safe, evidence-based and cost effective prescribing
  • Identify and manage medication-related risks and support prescribing safety initiatives.
  • Review hospital discharge information, medication changes and correspondence.
  • Provide specialist advice to clinicians on medicines, interactions, side effects and appropriate prescribing.
  • Support the management of high-risk medicines and prescribing alerts.
  • Develop and implement systems and processes to improve medicines safety and optimisation.
  • Contribute to clinical audits, quality improvement projects and practice development.
  • Liaise with the PCN, community pharmacies, secondary care and other healthcare professionals to ensure continuity of medicines management.
  • Contribute to the development and implementation of practice policies and procedures relating to prescribing and medicines management.
  • Provide leadership and support to pharmacy colleagues working within the practice.
  • Provide education and training to GPs, nurses, healthcare assistants, administrative staff and other members of the practice team regarding medicines and prescribing where appropriate.
  • Provide patients with clear and appropriate information regarding their medicines and treatment options.
  • Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved

We are a large teaching Practice of 15000 patients in Sunderland North.Our team consists of 3 GP Partners, Salaried GPs, ANPs, PNs, NAs, HCA, Phlebotomist, Clinical Pharmacist, Pharmacy Technician, the PCN Team and a large Admin Team.

As a teaching Practice we host medical students, GP trainees, pharmacy students, pre-registration pharmacists, nursing students and admin apprentices. We are also accredited for research.
