The Community Anchor role will enable individuals and communities to identify shared priorities, and support the development of locally led initiatives. The role will build stronger and resilient communities by empowering communities and developing bespoke activities to meet community need.

The Community Anchor role will use a relational approach to work with communities, the Voluntary Community Sector (VCS) and Key Delivery Partners (KDPs) to build capacity and resilience. The Community Anchor will empower communities to sustain activities to support the most disadvantaged and vulnerable. They will co-create offers and resources, such as listening walks to build stronger and resilient communities alongside promoting protective factors to health and well-being.

The Community Anchor will shape services based on specific geographies and communities, creating a bespoke offer across all localities within the Placed Based model.

Main duties of the job

To support the delivery of integrated place-based services by working collaboratively with KDPs and developing a joint Team around the Community Model which promotes community assets, increases social value and focuses on engagement from Children, Young People & Families (CYP&F)

Lead on the development of a Community Needs Profile and develop collaborative work streams with the community to address community need

The role will gather voice of the community using targeted approaches, to meet their needs and diversity, including forums, digital access, and feedback from individuals and KDPs. This will ensure service delivery reflects demographics, existing infrastructure and feedback to influence service improvement. Alongside other practitioners within the 0-19 Healthy Child Programme, the Community Anchor will be responsible for expanding outreach to vulnerable groups and strengthening community resilience. The Community Anchor will lead involvement of the community by supporting projects to enhance community assets

The role will develop place-based collaborative engagement strategies with KDPs and the community. All strategies will be co-created; outcome focussed and includes existing community networks and forums
