Job summary
Working with a multi-disciplinary team, you'll help coordinate care for people living with frailty so they receive the right support at the right time. You'll play an important role in keeping services running smoothly by organising meetings, coordinating actions and supporting effective communication between health, care and community partners. Your work will help ensure care plans are followed, information is shared accurately, and patients experience a coordinated journey through services. You'll be part of a supportive team that values collaboration, respect and person-centred care.
Main duties of the job
What you'll do:
- Coordinate and support multi-disciplinary team meetings, including preparing agendas and recording minutes
- Manage follow-up actions and help track progress against agreed care plans
- Build positive relationships with healthcare professionals, partner organisations, patients and carers
- Maintain accurate records, reports and data using digital systems
- Organise diaries, meetings and administrative processes that support efficient patient pathways
What we're looking for:
- Experience in an administrative or coordination role, ideally within health or social care
- Strong organisation, communication and relationship-building skills
- Confidence using Microsoft Office applications, databases and electronic records systems
- A careful approach to confidentiality, accuracy and attention to detail
- A relevant administration qualification, or equivalent knowledge and experience
