The Welwyn and Hatfield Integrated Community Team has an exciting opportunity for a Band 3 Health Care Assistant to join us.

Main duties of the job
Welwyn Hatfield Integrated Community Team are seeking a motivated and enthusiastic Health Care Assistant to join our friendly, supportive and award winning Integrated Care team.
You will be working alongside our trained nursing and therapy staff assisting them in the day to day care and management of our patients to ensure they receive safe, high quality nursing care.
As a Health Care Assistant, you will work alongside our diverse team comprised of nurses, paramedics, physiotherapists, occupational therapists and technical instructors. We work to provide evidence based nursing care to patients in their own homes.
Our service operates 8am - 10pm 365 days a year.
A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

In Welwyn and Hatfield Integrated Community team we have countless success stories of staff who have joined as a Health Care Assistant and gone on to complete the Nursing Associate training and Registered Nurse top up. We support our staff throughout their training, with time dedicated to training, university attendance and placements.

Job description
- To work within a team that provides an assessment and care service to patients with nursing / health needs.
- To visit patients in their own homes and provide appropriate care and support as defined in the Care Plan.
- The role will be undertaking wound care, administering medication and working jointly with nursing staff.
- To undertake administration and clerical tasks that are associated with the care process including record keeping and use of computer.
