The role of Coordination Centre Clinical Nurse Specialist is to be part of a small team of senior nurses who are responsible for the clinical coordination of care for patients and families referred to Rennie Grove Peace.

Main duties of the job

  • Being the point of contact for patients, families, clients and professionals who contact the hospice, dealing with all clinical enquiries.
  • Providing clinical oversight and triage for referrals to ensure that the patients and family members get the right Rennie Grove Peace service in a timely manner.
  • Developing the Coordination Centres nursing role to ensure it provides a responsive, evidence based and high-quality service to beneficiaries and professionals.
  • Working as a Specialist Palliative Care CNS in a different Rennie Grove Peace clinical setting on a monthly basis to maintain and develop skills.
  • The post holder will undertake activities such as audits and case review in order to improve the Coordination Centre Service.
  • Contributing to the wider strategic development of the hospice and help to translate local and national strategy relating to hospice care and Rennie Grove Peace to implement and deliver an effective service.
  • Act as a Rennie Grove Peace ambassador within the community.

Job description

  • Demonstrate clinical excellence and maintain consistently high standards of nursing and palliative care.
  • Act as a role model demonstrating high standards of care and providing clinical leadership for colleagues.
  • Advise and support on patients with complex needs and their families at home, maintaining an approach that incorporates physical, psychological, social and spiritual aspects of care, in accordance with the Rennie Grove Peace mission and values.
  • Effectively allocate and support team members to implement the care plan, monitoring and evaluating the care provided, and deliver practical nursing care when appropriate.
  • Ensure all written and verbal communication is constructive, effective, accurate and appropriate.
  • Be responsible for developing and maintaining effective working relationships with community professionals, support services and members of other hospices and palliative care teams, providing clinical expertise, specialist advice and support as requested.
  • Demonstrate a research-based approach to nursing and participate in on-going Rennie Grove Peace research.
  • Identify risk and lead risk management involved in patient care and lone working, implementing learning outcomes from recorded incidents.
  • Act as a mentor for new staff and oversee successful completion and timely sign off of their baseline competencies.
  • Carry out individual performance review process - annual and mid-year reviews - for any direct reports and complete personal development plans.
  • Participate fully in the orientation/mentorship of new staff.
  • Supervise and teach Rennie Grove Peace trained staff.
  • Provide specialist palliative care training to multi-disciplinary team colleagues and nursing hope colleagues within the community.

Our patient services are available to adults and patients transitioning into adult services (16 to 25 years.) Bereavement support is offered for all ages to families of patients and those around them.

The Charity is one of the larger UK hospice charities, reaching a GP population of 797,037 of whom 75% live in Hertfordshire and 25% in Buckinghamshire.

Our multidisciplinary team helps people to live well and receive support when they need it the most.
