Job summary

Cricklewood General Practice are looking for an experienced Practice Nurse to join our team.

The role will be 3 days a week, Monday, Tuesday and Wednesday, 09:00 to 17:00.

The salary is £21 to £25 depending on experience.

Main duties of the job

Our Practice Nurse will be responsible for public health and screening programmes, including infection prevention and control, chronic disease management, immunisations, health promotion and prevention, well man/woman clinics and cervical screening.

They will embed population health management approaches within general practice and target health inequalities.

This role is integral to the general practice team, as well as supporting the wider practice nurse network and multi-disciplinary teams.

We are looking for a candidate with previous experience working in a GP Practice.

Job description

Clinical Responsibilities

Act as the Infection Prevention and Control (IPC) lead for the practice, ensuring the IPC Policy is implemented and maintained

Consult with patients and their families to undertake assessments and deliver care based on patient need

Manage long-term conditions, contributing to quality improvement and assurance initiatives

Develop and embed an effective care management programme for frail and housebound patients

Support public health and screening programmes, including immunisations, vaccinations, and cancer screening

Perform Cervical screening

Provide opportunistic health promotion advice and lead on public health campaigns (e.g. flu clinics)

Provide travel advice services and vaccinations

Perform venepuncture and electrocardiography

Leg ulcer management, dopplers and compression bandaging.

Undertake wound care/ management/dressing techniques.

Carry out ear examination and syringing, following appropriate training and assessment.

Administer adult and paediatric injections under an individualised prescription or Patient Group Direction, ensuring safe storage, rotation and disposal of vaccines and drugs.

Provide routine nursing care in accordance with clinical guidelines

Chaperone patients where necessary and support the team in dealing with clinical emergencies

Liaise with external services and agencies to ensure appropriate support for vulnerable patients

Maintain chronic disease registers and implement an effective call/recall system

Maintain accurate clinical records using SNOMED CT codes and contribute to QOF targets

Professional Responsibilities

Recognise the boundaries of practice and refer patients to appropriate professionals as needed

Teach and advise patients and their families on self-management of their conditions

Participate in clinical audits, research, and the development and updating of practice protocols

Work within national and local protocols and contribute to clinical governance

Adhere to safeguarding duties for adults and children in line with current legislation

Participate in the management of patient complaints and learning from incidents and near-miss events

Undertake all mandatory training and attend a formal appraisal at least annually

Support the training and development of the nursing team, including HCA and GPA roles

Develop, review, and implement nursing protocols in conjunction with the practice manager

Participate in and support the coordination of the nursing team rota

Cricklewood General Practice is located in newly refurbished premises close to Cricklewood Station.

We currently have a patient population of 5,000 and serve an ethnically diverse urban patient population.
