Job summary
A first contact practitioner (FCP) is an experienced physiotherapist who has the advanced skills necessary toassess, diagnose and recommend appropriate treatment or referral for musculoskeletal (MSK) problems on a patient's first contact with healthcare servicese.g. when they visit their GP surgery. To progress and request investigations to facilitate diagnosis and choice of treatment.
Main duties of the job
Core Duties
Be the first point of clinical contact for patients presenting with MSK conditions or symptoms, providing the patient with the primary assessment, diagnosis and management options for their condition.
Use advanced clinical practice skills and clinical reasoning to provide comprehensive diagnostic and treatment support to patients who present with complex musculoskeletal conditions and/or multiple pathologies and/or mental health and/or pain management needs
Be the link between primary, community and acute services, when the patient is managed via primary care ensuring an integrated care pathway for individual patients that meets their needs. This will include provision of triage to other services and/or directly providing management and advice.
Provide leadership and support on MSK clinical and service development across the PCN, alongside learning opportunities for the whole multi-disciplinary team (MDT) within primary care.
Provide highly specialist advice on issues ranging from the provision of expert opinion on individual patient treatment options to be a primary contributor to MSK services and related pathway development.
Work independently, to assess, diagnose, triage, and manage patients, taking responsibility for prioritising and managing a caseload of the PCNs registered patients.

Primary Care Networks (PCN) are local neighbourhoods where all health and social care services work together to deliver a better experience for their citizens.
Synergy Health PCN covers three practices in Gedling Borough, Jubilee Park Medical Partnership in Carlton with a branch site in Lowdham, West Oak on Westdale Lane, Mapperley and Trentside Medical Group in Netherfield and Carlton.
We are a standalone PCN with a lead practice. We have a population of 36,000 patients.
The PCN has care co-ordinators, social prescribing link workers, advanced nurse practitioner, and clinical pharmacists. These roles will work closely together to develop and grow the PCN and improve patient care across our patient list.
Our vision is to build and develop collaborative working across member practices and stakeholders, to support and strengthen the delivery of Primary Care services and improve health outcomes for the local population we serve.
