The PCN Operations & Workforce Lead will play a key role in supporting the effective delivery of Primary Care Network (PCN) priorities through strong operational leadership, workforce coordination and project delivery.
Working closely with the Clinical Director, PCN Manager and Practice Managers, the postholder will lead on workforce planning, deployment and optimisation, ensuring that PCN resources are utilised effectively to meet the needs of patients, practices and the wider network.
Main duties of the job
The role will focus on the operational delivery of PCN services, workforce development, project implementation and performance monitoring, helping to ensure that network priorities are delivered efficiently and in line with local and national objectives.
The postholder will work collaboratively across member practices and with key stakeholders to support service improvement.
About the PCN
Phoenix Health PCN brings together three GP practices across Buckinghamshire working collaboratively to deliver more coordinated, accessible, and proactive healthcare for our local communities.
Our multidisciplinary team includes Health and Wellbeing Coaches, Clinical Pharmacists who support safe and effective medicines management, Pharmacy Technicians who assist with repeat prescribing and medication reviews, Care Coordinators who help patients navigate health and social care services, and Paramedics who provide timely assessment and urgent care within the community.
As demand grows and the needs of our population become increasingly complex, we are committed to strengthening our workforce further. We aim to expand our team with dedicated professionals who can help us provide tailored support to our most vulnerable patients such as those who may struggle with long-term conditions, frailty, social isolation, or barriers to accessing care. Job description
Key Responsibilities
Operations and Service Delivery
- Coordinate the operational delivery of PCN services and programmes.
- Support the implementation of PCN priorities and strategic objectives.
- Develop and maintain effective systems and processes to improve efficiency across the network.
- Monitor progress against agreed work plans and objectives.
- Identify operational risks and escalate issues appropriately.
- Support service development and quality improvement initiatives across member practices.
Workforce Planning and Development
- Lead workforce planning activities across the PCN.
- Coordinate the effective deployment and utilisation of ARRS-funded roles.
- Monitor workforce capacity, utilisation and productivity.
- Support recruitment, onboarding and retention initiatives.
- Work with practices to identify workforce gaps and opportunities for service improvement.
- Develop workforce reports and recommendations to inform decision-making.
- Support workforce development and training programmes.
Performance and Reporting
- Monitor delivery against PCN contractual requirements and agreed priorities.
- Analyse workforce, activity and service data to identify trends and opportunities for improvement.
- Support the development of performance dashboards and reporting frameworks.
- Provide assurance reports to the Clinical Director and PCN Board as required.
Project and Change Management
- Coordinate the implementation of new services, initiatives and projects.
- Support the delivery of transformation and service redesign programmes.
- Track project milestones, risks and outcomes.
- Facilitate collaboration between practices and stakeholders to support successful implementation.
- Promote a culture of continuous improvement across the network.
Essential
- Experience in healthcare, primary care or NHS settings
- Experience of workforce planning or operational management
- Strong project management and organisational skills
- Excellent communication and stakeholder management skills
- Ability to analyse and interpret performance data
Desirable
- Experience of working with GP practices or Integrated Care Systems.
- Knowledge of Primary Care Networks and PCN DES requirements.
