Job summary

We are looking to recruit an experienced and motivated Advanced Nurse Practitioner (ANP) to join our friendly and supportive GP practice team. This is an exciting opportunity for a highly skilled clinician to work autonomously within primary care, delivering high-quality, patient-centred care.

The successful candidate will assess, diagnose, and manage a wide range of acute and chronic conditions, working as part of a multidisciplinary team to support the delivery of effective and accessible services to our patient population.

Main duties of the job

  • Provide advanced clinical assessment, diagnosis, and treatment for patients presenting with undifferentiated conditions
  • Independently prescribe medication in line with current guidelines and best practice
  • Manage same-day appointments, minor illness, and urgent presentations
  • Undertake clinical management of patients with long-term conditions where appropriate
  • Work collaboratively with GPs, practice nurses, pharmacists, and wider PCN teams
  • Contribute to clinical audits, quality improvement, and QOF delivery
  • Support triage systems and improve patient access to care
  • Maintain accurate and contemporaneous patient records using clinical systems (EMIS)
  • Participate in practice meetings and contribute to service development

Job description

Key Responsibilities

The Advanced Nurse Practitioner (ANP) will act autonomously within their professional scope of practice, providing high-quality clinical care to patients registered at the practice. The post holder will be responsible for assessment, diagnosis, treatment, and evaluation of care for patients presenting with a wide range of undifferentiated and undiagnosed conditions.

The ANP will work collaboratively with the multidisciplinary team to support safe, effective, and responsive primary care services.

Clinical Practice

  • Provide expert clinical assessment, diagnosis, and treatment for patients with acute and chronic conditions
  • Manage patients presenting with undifferentiated and undiagnosed illnesses
  • Prescribe medications independently in accordance with prescribing regulations and practice protocols
  • Undertake clinical decision-making and risk management in complex situations
  • Deliver evidence-based care in line with national and local guidelines
  • Refer patients appropriately to secondary care or other services

Patient Care & Service Delivery

  • Support same-day access clinics and triage systems
  • Contribute to improving patient access and flow within the practice
  • Participate in the management of long-term conditions where appropriate
  • Promote health education and self-management to patients

Team Working

  • Work collaboratively with GPs, nurses, pharmacists, and administrative staff
  • Participate in multidisciplinary team meetings
  • Provide clinical advice and support to colleagues where appropriate
  • Contribute to the development of integrated working within the Primary Care Network (PCN)

Leadership & Service Development

  • Contribute to service improvement initiatives and practice development
  • Participate in clinical audit, quality improvement, and governance activities
  • Support the achievement of Quality and Outcomes Framework (QOF) targets
  • Assist in the development and implementation of practice policies and protocols

Education & Training

  • Maintain own professional development and revalidation requirements
  • Participate in clinical supervision and appraisal processes
  • Support the training and mentorship of junior staff and students where appropriate

Governance & Compliance

  • Maintain accurate, contemporaneous patient records in line with legal and professional standards
  • Adhere to NMC Code of Conduct and practice policies
  • Ensure compliance with safeguarding, infection control, and information governance requirements
  • Participate in significant event reviews and incident reporting

Professional Responsibilities

  • Work within the scope of professional practice and competencies
  • Maintain registration with the Nursing and Midwifery Council (NMC)
  • Ensure continued professional development in line with role requirements
  • Act as a role model for high standards of clinical care and professionalism

General Duties

  • Contribute to a positive and supportive working environment
  • Maintain confidentiality at all times
  • Undertake any other duties appropriate to the role as delegated by the Practice Manager or GP Partners
