We are seeking an experienced and ambitious PCN Manager to provide strategic leadership, governance and organisational support to our Primary Care Network (PCN). Working closely with the Clinical Director, PCN Board and member practices, you will play a pivotal role in shaping the future direction of the Network and ensuring the successful delivery of its strategic priorities.

This is an exciting opportunity for a skilled Primary Care or NHS leader who can bring strategic insight, strong governance expertise and a collaborative approach to support the ongoing development of the PCN and the services we provide to our population.

Main duties of the job

This is a flexible, part-time position ideally suited to an experienced Primary Care or NHS Manager who can provide strategic oversight, organisational leadership and governance while supporting the operational leadership provided by the PCN Operations & Workforce Lead.

About the PCN

Phoenix Health PCN brings together three GP practices across Buckinghamshire working collaboratively to deliver more coordinated, accessible, and proactive healthcare for our local communities.

Our multidisciplinary team includes Health and Wellbeing Coaches, Clinical Pharmacists who support safe and effective medicines management, Pharmacy Technicians who assist with repeat prescribing and medication reviews, Care Coordinators who help patients navigate health and social care services, and Paramedics who provide timely assessment and urgent care within the community.

As demand grows and the needs of our population become increasingly complex, we are committed to strengthening our workforce further. We aim to expand our team with dedicated professionals who can help us provide tailored support to our most vulnerable patients such as those who may struggle with long-term conditions, frailty, social isolation, or barriers to accessing care. Our goal is to enhance continuity of care, improve patient outcomes, and ensure that everyone receives the right care at the right time.

Job description

Key Responsibilities

Strategic Leadership

  • Support the Clinical Director and PCN Board in delivering the Network's strategic objectives.
  • Provide professional advice on operational, governance and organisational matters.
  • Support the development and review of the PCN's annual business plan and work programme.
  • Ensure that agreed priorities are translated into clear operational actions.
  • Identify opportunities to improve efficiency, collaboration and service delivery across the Network.

Governance and Board Support

  • Attend and support PCN Board meetings.
  • Prepare agendas, reports and supporting documentation in conjunction with the Clinical Director.
  • Ensure accurate minutes and action logs are maintained.
  • Monitor progress against Board decisions and agreed actions.
  • Maintain governance documentation, policies and organisational records.
  • Support compliance with the PCN DES, national guidance and local contractual requirements.
  • Maintain oversight of the PCN risk register and ensure risks are appropriately escalated.

Line Management and Professional Support

  • Provide line management for the PCN Operations & Workforce Lead.
  • Hold regular one-to-one meetings to review progress, priorities and wellbeing.
  • Support objective setting, performance development and annual appraisal.
  • Provide mentoring, coaching and professional guidance.
  • Support the resolution of operational challenges and remove barriers to delivery.
  • Escalate significant issues to the Clinical Director where appropriate.

Performance Oversight

  • Review delivery against the PCN's objectives, contractual requirements and work plan.
  • Monitor key performance indicators and operational reports.
  • Provide assurance to the Clinical Director and Board regarding progress and delivery.
  • Identify areas of risk or underperformance and work with the Operations & Workforce Lead to develop improvement plans.
  • Ensure appropriate reporting arrangements are in place.

Financial Oversight

  • Support oversight of PCN budgets and commissioned funding.
  • Review financial reports and expenditure with the finance team.
  • Support planning for investment of PCN resources.
  • Ensure resources are used effectively and in line with agreed priorities
