Job summary

Working closely alongside our GP team, the successful candidate will play a key role in managing acute on-the-day demand and supporting patients within the community.

Due to increasing demand for same day appointments and community-based care, we are seeking an experienced and motivated Advanced Nurse Practitioner (ANP) to join our clinical team for four sessions per week.

Main duties of the job

Key responsibilities will include:

  • Conducting home visits for housebound and clinically appropriate patients.
  • Managing on-the-day acute presentations.
  • Assessing, diagnosing and treating patients independently within their scope of practice.
  • Supporting and managing triage requests through our newly implemented Klinik online consultation and triage system.
  • Providing clinical advice and signposting patients to the most appropriate service.
  • Prescribing independently.
  • Maintaining accurate and timely clinical records.
  • Working collaboratively with GPs, practice nurses, administrative staff and wider healthcare professionals.

We are also a Training Practice and normally support 2 GP Registrars

Non Clinical Team, consists of Reception, Prescribing Team, Letter Readers and a Secretary
