Main duties of the job

As a Clinical Pharmacist, you will play a key role within our multidisciplinary primary care team, providing expert clinical pharmacy services to improve patient outcomes and support safe, effective prescribing. You will undertake structured medication reviews, manage long-term conditions, optimise medicines, and provide specialist advice to patients and healthcare professionals. Working closely with GPs, nurses, community pharmacies and secondary care, you will help ensure seamless medicines management across care settings. The role also includes supporting quality improvement initiatives, promoting evidence-based practice, contributing to service development, and providing education and support to colleagues. Job description

The following key responsibilities in delivering health services at Newgate Medical Group.There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

Primary responsibilities:

  • Provide patient-facing clinical pharmacy services as part of the multidisciplinary team.
  • Assess, treat and prescribe within professional competence.
  • Carry out structured medication reviews and optimise medicines for patients with long-term conditions and complex needs.
  • Support medicines optimisation, antimicrobial stewardship and quality improvement initiatives.
  • Promote safe, effective and cost-efficient prescribing.
  • Manage repeat prescribing, medication monitoring and high-risk drug recalls.
  • Respond to medicines-related queries from patients and healthcare professionals.
  • Conduct medication reviews for newly registered patients, care home residents and housebound patients.
  • Deliver long-term condition clinics and pharmaceutical consultations.
  • Work collaboratively with GPs, community pharmacies, hospitals and wider healthcare teams.
  • Lead on medicines management processes and prescribing systems.
  • Maintain accurate clinical records and comply with professional, legal and safeguarding requirements.
  • Support clinical audit, service improvement and implementation of national guidance (including NICE).
  • Supervise the safe storage, handling and disposal of medicines and vaccines.
  • Participate in clinical supervision, mandatory training, appraisal and continuing professional development.
  • Contribute to enhanced services, public health initiatives and clinical governance.
  • Provide leadership, education and support to colleagues on medicines-related matters.
  • Undertake other duties appropriate to the role as required.

In addition to the primary responsibilities, the Clinical Pharmacist will:

  • Support the delivery of QOF, QIPP, incentive schemes and other quality improvement initiatives.
  • Review prescribing formularies, protocols and compliance.
  • Improve the quality of medicines data and clinical coding.
  • Deliver training, mentoring and medicines-related guidance to colleagues and students.
  • Work with hospital and community pharmacy teams to improve medicines safety and continuity of care.
  • Provide leadership and support to prescription and dispensary staff.
  • Develop and share medicines information through newsletters, bulletins and education.
  • Develop and maintain a specialist area of clinical expertise.
  • Support digital models of care, including remote consultations and medication reviews.
  • Undertake duties appropriate to the role and contribute to the effective delivery of the service.
