Job summary

What are we looking for? Mindworks Community Teams in North Surrey are looking for passionate Registered Mental Health Practitioners, Occupational Therapists, and Social Workers who are enthusiastic to come and work at a friendly, committed and established Children and Young People's Services (CYPS) Mental Health Service where your knowledge and skills will be valued.

As part of a busy CYPS Directorate and alongside our Alliance Partnership including local voluntary sector organisations, you will be ensuring children, young people and families are at the centre of decision making to promote person centred care and outcomes, you will support the Community Team in the implementation of the getting help and getting more help elements of the ITHRIVE framework.

We are looking for clinicians who are creative, highly motivated, show initiative and are passionate and committed for ensuring high quality services for young people with mental health difficulties, along with the relevant leadership and supervision skills.

Excellent opportunities will be available for training and professional development both locally, within SABP network and across the entire alliance including the Tavistock and Portman

Main duties of the job

The post holder will be a member of the multi-disciplinary specialist team, providing services to children and young people 6-18 years and young people in transition to adult services with mental health needs, emotional and behavioural problems whilst also providing leadership to our Mindworks community team.

The main responsibilities of the post holder will be providing assessments and treatment within our Mindworks Community Teams in North either individually or with a member of the multidisciplinary team depending on the complexity of the referral, both in clinic and community settings. The post holder will be able to offer a range of therapeutic interventions appropriate to the care pathway the young person is on.

As part of a busy CYPS Directorate and alongside our Alliance Partnership including local voluntary sector organisations, you will be supporting the team in leading with the implementation and embedding of the THRIVE Framework across our newly created Mindworks Community Team in Surrey.

Job description

The post holder will work closely with the Mindworks Alliance including School Based Needs, Surrey Well Being Partnership and other agencies during office hours, responding to referrals of young people presenting in need:

The post holder will work as an autonomous practitioner, undertaking clinical assessment of children and young people who may have a complex co-morbid mental health need, working alongside the wider network and providing consultations where necessary

To be an integral part of the Hub duty Rota, responding to queries from families and the wider network including risk support.

To work within a highly experienced multi-disciplinary team (MDT) involved in the assessment and treatment of children and young people with serious and complex mental health difficulties.

To manage own caseload from referral to discharge/transfer, holding medium to high-risk cases in liaison with the MDT.

Ensure that outcome measures are implemented and audited. Contribute to relevant research and ensure that interventions on the pathway are evidence based.

Be responsible for the oversight and completion of initial assessment and subsequent facilitation of a warm handover and referrals to other services and/or discharge with a signed care plan regarding safety and recorded on the electronic patient record.
