Rowhedge & University of Essex Medical Practiceis currently looking for a Social Prescriber to join our team.The Social Prescriber will play a vital role in enhancing patient care and well-being by connecting individuals with non-medical support services within the community.

Main duties of the job

• Collaborate with healthcare professionals to identify patients who would benefit from social prescribing.
• Conduct holistic assessments to understand patients' social needs and preferences.
• Develop personalized care plans and signpost patients to relevant community resources and services.
• Educate patients on available social support options and empower them to take control of their health.
• Organize and facilitate group activities that promote mental well-being, physical health, and social inclusion.
• Maintain accurate records of patient interactions and outcomes in accordance with data protection regulations.
