Job summary
This new role will add something extra to our existing team. The main duties will include working as one of the Practice team of GPs and Nurses delivering general practice Services to non-registered and registered patients for Hove Medical Centre HMC with particular emphasis on frail elderly/housebound patients including those in nursing/care homes. At all times you must act in a manner consistent with the code of conduct and appearance representing HMC.
Main duties of the job
MAIN DUTIES, RESPONSIBILITIES AND CLINICAL DUTIES
- Housebound visits as directed by the Practice Manager
- Ward Rounds in Nursing Homes weekly as directed by GP/Practice Manager
- Home visits as required
- Chronic disease review on housebound patients as directed by GP/Practice Manager
- Daily telephone advice as directed by GP.
- Management of patients with chronic diseases particularly Dementia, diabetes and respiratory medicine in Nursing Homes/Housebound as directed.
- Support other members of the Practice team as required
- Liaise with GPs for their Frailty lists reporting to the Practice Manager
- Any other duties required for this role

MANAGEMENT AND ADMINISTRATION
- Participate in meetings as required
- Liaise with multi-disciplinary teams when necessary & collate patients
- To keep up to date with relevant, necessary update training
- Recording of QOF data in patient records as appropriate
- To perform audits for the Practice if required
- Liaise with GPs/Practice Manager and abide by their instruction
- Liaising effectively with the whole Practice team
- Following referral pathways as set by the local ICB
- To ensure that membership to professional bodies is kept up to date
GENERAL DUTIES
- To keep up to date with new clinical guidance
- To be punctual
- To adhere to relevant health and safety procedures
- To follow both practice and local policies and procedures
- To comply with Practice Policies/Protocols both clinical and administrative
