An exciting replacement rolehas opened for a motivated, professional, and caring Practice Nurse with a Diabetes qualification to join our supportive multi-disciplinary team. Working across our two site practice, we provide high-quality care to a diverse population of 16,000 patients.

We are looking for a dedicated professional who thrives in a team environment and can demonstrate excellent, patient-centred care with compassion and professionalism. The successful candidate will work alongside the rest of the clinical team to manage chronic disease, while also supporting general practice nursing duties, using SystmOne for all clinical record-keeping.

Main duties of the job

  • Collaborative Diabetes Care: Work alongside our current Diabetes Nurse to deliver comprehensive assessment, treatment, and lifestyle advice for patients with diabetes.
  • General Nursing Care: Deliver core treatment room services, including but not limited to; immunisations, cervical screening, and wound management.
  • Clinical System Utilization: Maintain accurate, comprehensive, and contemporaneous patient records using SystmOne.
  • Multidisciplinary Collaboration: Work collaboratively within our multi-disciplinary team (including GPs, ANPs, Practice Nurses, TNA, HCAs and AHPs) to support patient care pathways
  • Quality Frameworks: Support the practice in achieving the Quality Outcomes Framework indicators and local quality initiatives related to diabetes.

Job Summary

The post holder, acting within their professional boundaries, will provide a high standard of nursing care for MMC patients, including clinical assessment, treatment, and evaluation of care. They will undertake all duties usually expected within the Practice Nurse role, alongside additional responsibilities reflecting their extended skills in long term condition management and independent prescribing within their scope of competence.

The post holder will work collaboratively with the general practice team to meet the needs of patients, support the delivery of policy and procedures, and contribute to high quality, evidence based care. They may be required to work across MMC practices and the PCN where necessary.

Key Responsibilities

Clinical Practice

Core Practice Nurse Duties

Holistic clinical assessment and management of patients within scope of practice.

Support patients to adhere to prescribed treatments and self management plans.

Childhood and adult vaccinations under PGDs.

Cervical cytology and smear testing.

Wound care and dressings.

Phlebotomy.

Learning disability health checks.

Contraception advice and monitoring.

General health checks.

ECG recording.

Administration of injections.

Ear care.

Obtain and document informed consent and capacity assessments.

Ensure infection control standards are maintained.

Monitor and maintain clinical stock and equipment.

Recognise and respond to emergency conditions such as anaphylaxis and cardiac arrest.

Promote health education and selfcare strategies.

Home visits for chronic disease monitoring and vaccination delivery.

Maintain uptodate clinical skills and attend nursing team meetings.

Comply with MMC policies and procedures.

Extended Skills Responsibilities

The post holder will undertake additional duties aligned with their extended competencies:

Long term condition (LTC) management, including structured reviews for conditions such as asthma, COPD, diabetes, hypertension, and cardiovascular disease.

Medication titration within scope of competence, utilising local guidelines.

Independent prescribing within scope of competence, ensuring safe, evidence based prescribing practice.

Targeted clinical examination relevant to LTC management within scope of competence.

Contribute to personalised care planning for patients with chronic conditions.

Provide clinical advice to colleagues within defined boundaries.

Communication

Use sensitive and effective communication to ensure patients are fully informed and able to consent to treatment.

Communicate with and support patients receiving difficult or distressing information.

Adapt communication methods to meet individual needs, including cultural, cognitive, or language barriers.

Maintain effective communication within the practice team to support highquality service delivery.

Act as an advocate for patients and colleagues.

Delivering a Quality Service

Work within professional competence and the NMC Code to deliver evidence based care.

Comply with NMC revalidation, appraisal processes, and mandatory training.

Produce accurate, contemporaneous clinical records in line with legislation and practice policies.
