1 x Fixed Term contract for up to 6 months with the potential to extend.

This post is part-time 22.5 hours per week.

This is a very exciting opportunity to join a small multi-disciplinary team working with people experiencing rough sleeping with outstanding complex needs. The role of peer support worker has been developed specifically for people who have lived experience of complex needs and homelessness.

The CHPSS team is made up of 1x Team Manager, 2x Mental Health Nurses, 1x Adult Social Care Navigator, 1x Peer Support Worker, 1x Psychologist, 1x assistant psychologist, 1x Occupational Therapist, 2x VIA Recovery Practitioners and 1x administrator.

The core key responsibilities include:

Working as part of a team supporting people with multiple complex needs who are also experiencing or at risk of street homelessness.

To work across systems within GHC and across organisation, including Housing, Adult Social Care (ASC), VIA substance misuse service, and other relevant partners.

To work in unconventional clinical environments (e.g. rough sleeper sites, supported housing, partner agency units, in the community etc) to deliver support to individuals with complex emotional needs.

The successful candidate would be a highly valued member of the multi-disciplinary team, the Peer Support Worker will offer support and practical assistance to service users to support their recovery and promote independence.

Main duties of the job

To communicate with the multi disciplinary team, wider professionals and external agencies, keeping accurate and complete records consistent with the Trust's policies and procedures.

To act as a specialist resource within the service, by offering a lived experience perspective to service users. To act as an expert on recovery resources.

To facilitate positive culture change within the workplace.

To share ideas about ways of achieving recovery goals, supporting individuals from a holistic perspective.

When required, accompany service users to appointments, meetings and activities aligned to their recovery goals.

Identify risk and ensure appropriate risk/safety assessments are carried out, and comply with Health and Safety requirements.

To attend and actively participate in MDT meetings, supervision meetings and other internal/external meetings as agreed.

Make changes in own practice and offer suggestions for improving services and to contribute to the ongoing development of the CHPSS Team.

To maintain good communication with colleagues within Gloucestershire Health and Care NHS Foundation Trust, peer mentors, partner organisations and external agencies.

To adhere to the trust clinical guidelines, code of ethics, policies, procedures and stated aims / core purpose.

Job description

To establish and maintain effective links with internal/external agencies.

To take proactive steps to ensure service accessibility. This includes joint assertive outreach with Julian House and leading engagement groups across the county.

To offer practical support to service users, joint working alongside the lead professional.

Sharing personal lived experience, which may include revisiting distressing life events

Managing professional boundaries while drawing on personal experiences

Navigating multi-agency systems and overcoming barriers to service access

Organise and prioritise workload in an environment with interruptions and changing prioritise.
