Job summary
Are you passionate about supporting children and families facing the challenges of a serious or life-limiting condition Demelza is seeking a compassionate and proactive Family Liaison Practitioner In-Reach to work alongside our NHS partners and help ensure that babies, children, young people and their families receive the right support at the right time.
Main duties of the job
Based across NHS Trusts and Demelza sites, you will build strong relationships with healthcare professionals, raise awareness of Demelza's services, support referral pathways and provide practical and emotional support to families. Working closely with our Nursing, Care and Family Support teams, you will help improve access to services and ensure families receive coordinated, family-centred care.
Job description
To work with Service Development and the Community Nursing Teams to research and identify gaps in referral sources, raising Demelzas profile, meeting strategic objectives by increasing referral numbers. To engage with potential referrers to raise awareness of range of support available to families, to be familiar with Demelzas Guide to Services, offer information and regular presentations to keep relevant staff up to date. To work with families to raise awareness of Demelza and support available, directed by and be knowledgeable of the neonatal pathway in place within Kings NICU and Oliver Fisher Unit
