Consultant / Specialist Doctor in Frailty and Hospital at Home

An exciting opportunity has arisen to join our Frailty and Hospital at Home services at Tameside and Glossop Integrated Care NHS Foundation Trust. We are seeking a Consultant or a Specialist Doctor in Frailty, Older People's Medicine or Community Geriatrics to provide senior clinical leadership across the Acute Frailty Hub, Frailty Same Day Emergency Care (SDEC), Hospital at Home and Virtual Ward services.

The Acute Frailty Hub provides comprehensive geriatric assessment, rapid specialist review and multidisciplinary care for older adults with frailty and complex health needs. Alongside this, our Hospital at Home and Virtual Ward services deliver hospital-level care in patients' homes through remote monitoring, digital technology and community support.

Working alongside Consultants, GPs, Advanced Clinical Practitioners, Nurses, Therapists, Pharmacists and Social Workers, you will provide expert assessment, diagnosis and management for patients with frailty, long-term conditions and acute illness. The role combines direct clinical care, multidisciplinary leadership, service development and support for admission avoidance and early discharge pathways.

This is an excellent opportunity to help shape integrated frailty services across acute and community settings while supporting innovative care closer to home and improving outcomes for older people.

Main duties of the job

  • Provide senior clinical leadership across the Acute Frailty Hub, Frailty SDEC, Hospital at Home and Virtual Ward services.
  • Undertake comprehensive geriatric assessment, diagnosis and management of older adults with frailty, multi-morbidity and acute medical presentations.
  • Deliver senior clinical decision-making to support admission avoidance, early discharge and care closer to home.
  • Lead multidisciplinary team meetings, complex case reviews, treatment escalation planning and discharge decision-making.
  • Provide clinical care across acute, ambulatory, virtual and community settings, including frailty and Hospital at Home pathways.
  • Manage patients with frailty syndromes, long-term conditions, rehabilitation and end-of-life care needs.
  • Work collaboratively with GPs, ACPs, Nurses, Therapists, Pharmacists, Social Workers and wider system partners to deliver integrated, person-centred care.
  • Support the development of frailty and community-based pathways, promoting innovation and service improvement.
  • Participate in clinical governance, audit, quality improvement, patient safety reviews and complaints management.
  • Provide supervision, mentorship and education for junior doctors, ACPs and multidisciplinary colleagues.
  • Develop effective partnerships across primary, community, acute and intermediate care services to improve patient outcomes.
  • Demonstrate and uphold the Trust's CARE values, promoting compassionate, respectful and inclusive care.
