Towcester Medical Centre is a busy and forward-thinking GP practice serving a growing patient population. We are looking for an enthusiastic, caring and motivated Practice Nurse to join our established clinical team.
This is an excellent opportunity for an experienced Practice Nurse, or someone looking to develop their career within general practice.
Main duties of the job
The role
You will work as part of our multidisciplinary team, providing high-quality nursing care to our patients. The role will include a range of general practice nursing duties, which may include:
- Chronic disease management
- Childhood and adult immunisations
- Cervical screening
- Wound care and dressings
- Blood pressure and other routine observations
- Health promotion and patient education
- Supporting patients with self-management of their health
- Working collaboratively with GPs, healthcare assistants and the wider practice team
Job description
- Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being
- Implement and evaluate individual treatment plans for patients with a known long-term condition
- Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition
- Management of patients with diabetes and other chronic diseases
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care
- Support patients to adopt health promotion strategies that promote patients to live healthily, and encourage principles of self-care
- Assess and care for patients presenting with wounds
- Support and advise women requesting information relating to family planning needs
- Support and manage health needs of women presenting for cervical cytology consultations
- Implement and participate in vaccination and immunisation programmes for both adults and children
- Advise, support and, where appropriate, administer vaccinations for patients travelling abroad
- The post holder may be required to provide services to the Practices housebound patients in their own homes to ensure equitable access to primary care services.
