Job summary
We are looking for 2 full time (37.5 hours) and 2 part time (18.75 hours) Family Floating Support Practitioners to join our team,providing flexible, trauma informed and strengths based support to families experiencing homelessness, housing instability or multiple disadvantage.
You will work holistically with adults and children, helping families secure and sustain long-term accommodation, improve wellbeing, strengthen routines and build independence. Working across Bradford, you will provide personalised support in homes, community venues and partner sites, working closely with Housing Options, Family Hubs, Children's Social Care, schools, health services and VCSE partners.

Main duties of the job
- Hold a caseload of approximately 10 families.
- Provide person centred, trauma informed floating support to families.
- Complete holistic assessments and develop personalised Independence Plans.
- Support families to secure and sustain suitable long-term accommodation.
- Deliver tenancy sustainment support, including budgeting, routines and income maximisation.
- Build trusting relationships using strengths based and trauma informed approaches.
- Adapt communication and support for neurodivergent adults and children.
- Work collaboratively with housing, health, social care and voluntary sector partners.
- Attend multi-agency meetings and contribute to safeguarding processes where required.
- Maintain accurate records and contribute to service monitoring and reporting.
- Promote health, wellbeing and independence through every contact.

Ensure support is culturally responsive, neuro-inclusive and accessible.
Key Responsibilities
Main Tasks
Provide personcentred support that empowers families to make informed choices.
Complete holistic assessments exploring housing history, risks, strengths, aspirations, routines, finances and wellbeing.
Develop personalised Independence Plans and review progress regularly.
Support families to secure suitable longterm housing, including social housing, private rented and specialist options.
Deliver practical tenancy sustainment support (budgeting, routines, income maximisation, problemsolving).
Communicate complex or sensitive information with compassion and clarity.
Use trauma-informed approaches to build trust, emotional safety and engagement.
Adapt communication and support for neurodivergent adults/children.
Attend multiagency meetings (Early Help, CIN, CP, MARAC, MAPPA).
Maintain accurate records and contribute to monitoring and reporting.
Promote every contact counts and encourage families to talk about health and wellbeing.
Communication and Working Relationships
Required to:
Build strong relationships with families, housing providers, social care, health partners and VCSE organisations.
Work collaboratively to reduce barriers and prevent tenancy breakdown.
Special Working Conditions
Work across Bradford district, including homes and community venues.
Occasional irregular hours to meet family needs.
