Job summary

We are seeking a dedicated and skilled Social Worker to join our Social Work and Community Support team on a part-time basis. This is a rewarding opportunity to make a meaningful difference to the lives of adults, older people, carers and families within the Jewish community. You will provide professional assessment, care planning, advocacy and safeguarding support, helping people to maintain their independence, wellbeing and connection to their communities. Working as part of a friendly and supportive multidisciplinary team, you will manage a varied caseload and work with people experiencing challenges associated with ageing, ill health, disability, dementia, mental health needs and social isolation. You will work collaboratively with individuals, families, colleagues and external agencies to identify strengths, manage risks and develop person-centred plans that promote dignity, choice and positive outcomes. The role also offers opportunities to contribute to community initiatives, partnership working and the ongoing development of the Social Work and Community Support Service. This is a permanent, part-time position working 18 hours per week. The role involves travel across Hertfordshire and Greater London, and access to a vehicle for work is essential.

We are looking for a dedicated and skilled Social Worker to join our Social Work and Community Support team.

Our Social Work and Community Support Service (SWCS) delivers personalised, integrated support to individuals, families, and carers across multiple regions.

About the Role In this impactful role, you will work with individuals and carers facing challenges such as ageing, ill health, disability, mental health needs, or social isolation. You will provide professional social work support, including assessment, care planning, advocacy, and safeguarding, enabling people to maintain independence and improve their quality of life.

You will also play a key role in developing services, contributing to community initiatives, and representing Jewish Care within multi-disciplinary partnerships.

This position will require travel, and its is essential you have a vehicle for work covering Herts and Greater London area.

This post is part time for 18 hours per week.

Key Responsibilities

  • Manage a caseload of approximately 35 cases (FTE), delivering high-quality, person-centred social work interventions
  • Undertake holistic assessments, care planning, and risk management
  • Work in partnership with individuals, carers, and statutory and voluntary services
  • Provide advice, information, and advocacy in line with current legislation and guidance
  • Play an active role in multi-disciplinary working and partnership development
  • Support the development of community and group-based interventions
  • Promote independence, choice, and dignity for individuals and families
  • Represent the service at meetings, forums, and community settings
  • Maintain accurate and timely case records using the organisations CRM system
  • Contribute to the development and continuous improvement of services
  • Triage and respond to referrals, assessing urgency and need
  • Apply professional judgement to make timely, effective decisions
  • Maintain an up-to-date CPD portfolio and engage in ongoing professional development

About You We are looking for a compassionate, confident, and reflective practitioner who is committed to delivering excellent social work practice.

Essential:

  • Qualified Social Worker with current registration with Social Work England
  • Experience working with adults in a social care setting
  • Strong experience supporting people with mental health needs, disability, dementia, or complex needs
  • Sound knowledge of Safeguarding, the Care Act (2014), and the Mental Capacity Act (2005)
  • Experience of community and/or group work
  • Excellent communication, assessment, and IT skills
  • Ability to manage a complex caseload and make evidence-based decisions
  • Strong understanding of professional ethics, confidentiality, and safeguarding responsibilities
  • Ability to work both independently and as part of a multi-disciplinary team
  • Commitment to learning about and engaging with the Jewish community and culture

This position will require travel, and its is essential you have a vehicle for work covering Herts and Greater London area.

Desirable:

  • Experience working within the voluntary sector
  • Experience in mental health services
  • Knowledge of the Holocaust survivor community and related needs
  • Awareness of Claims Conference processes
