Job summary
To improve care and health outcomes for patients supporting patients with medication reviews and to self-manage their well-being and long term conditions, through optimising medicines and provision of patient facing clinical pharmacy services. The practice will actively support pharmacists to work towards CPPE and independent prescribing status.
Main duties of the job
The post holder will:
Provide face to face structured medication reviews for patients.
Manage patients with long term conditions.
Manage of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, acute prescription request.
Contribute to the development of the prescribing and medicines management strategy and systems throughout the practice.
Undertake clinical audit, review, monitoring and instigating change in primary care prescribing.
Act as a credible and independent source of expert pharmaceutical advice to promote high quality evidence based cost-effective prescribing within safe management systems in primary care.
Job description
- The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation and acute prescription request.
- The post holder will perform face to face medication reviews of patients including polypharmacy reviews especially for older people, people in residential care homes and those with multiple co-morbidities.
- The post holder will engage with quality improvement and clinical audit and well as managing some aspects of the Quality and Outcomes Framework.
- This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice.
- The practice can provide support and training for pharmacists new to general practice.
