We are seeking a motivated and experienced Practice Nurse with general practice experience to join our team.
The role requires independent clinical decision-making, with support from GPs and the wider multidisciplinary team when required. The successful candidate will be adaptable, proactive, and committed to delivering safe, evidence-based care.
There will be a need to work collaboratively with the general practice team to meet the needs of the patients, ensuring that practice policies and procedures are followed.

Sleaford Medical Group is seeking a motivated and experienced Practice Nurse to lead and deliver high-quality patient care. Working within a well-established, patient-focused GP practice, the post holder will play a key role in delivering evidence-based care in line with QOF requirements and national guidelines.
The role requires clinical autonomy, with the ability to make independent decisions while working collaboratively with GPs and the wider multidisciplinary team. The successful candidate will be adaptable, forward-thinking, and committed to providing safe, effective, and patient-centred care to meet the needs of the local population.
Main Duties of the Role
Clinical Responsibilities
- To work within clinical boundaries, demonstrating safe clinical decision-making, delivering high quality clinical care.
- To provide essential treatment duties such as BP checks, Long Term Condition Reviews, NHS Healthchecks, prescribed injections, BMI and lifestyle advice
- Provide holistic assessment of wounds and implement wound care including complex wound management in line with current evidence-based guidelines
- Offer advice about childhood and adult immunisations and ensure vaccines/injections are administered under patient group directives (PGDs) or individual prescriptions (PSDs)
- Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule
- Actively participate in seasonal and special projects as requested e.g. flu campaign
- Act as chaperone when required
- Vaccine / cold chain storage, monitoring and recording
- Undertake health promotion to help educate patients
- To maintain accurate patient records and enter onto the computer using agreed Read Codes
- To collect data and participate in regular Practice audit and research projects
- To contribute to the preparation of any Practice development plans
- To maintain own annual registration with the NMC and adhere to Code of Professional Conduct
- To maintain own continuous professional development with the support of the practice as required
- Attend and participate in staff and Practice meetings and clinical training sessions.
