Community Hospice at Home Senior Staff Nurse/Paramedic
- Hours: 34.5 hours per week (3 shifts of 11.5 hours per week)
- Location: Rennie Grove Peace Watford & Hertsmere Locality
- Salary: Rennie Grove Peace Band 6 - £38,682 - £46,580 per annum (pro rata)

Main duties of the job
- Proactively contribute to the delivery of high-quality safe evidence-based care.
- Responding to SOS calls visiting patients in their own home to provide palliative and end of life care.
- To lead on and coordinate assessment, planning and evaluation of individualised patient centred care.
- Ensuring a holistic approach that is sensitive and responsive to the patients ever changing needs.
- To be responsible for the coordination and smooth running of a shift and visits ensuring it is well organised.
- As a member of the wider Rennie Grove Peace Care staff sign up and demonstrate delivery of the organisational agreed mission, vision and values.
- Acting as a Rennie Grove Peace ambassador within the community.
- As a lone worker be responsible for your own and others safety by adhering to the Lone Worker policy and procedure requirements.
Job description
- To undertake holistic needs assessment that explores individual patients and their carers/families physical, emotional spiritual, practical, welfare right and if appropriate the families pre and post bereavement need, using a variety of tools to support the process.
- To be able to develop, update and monitor the individualised care plans ensuring they are patient centred and support the identified patient needs, including ongoing referrals to other services as required.
- To be able to deliver both generalist basic and complex specialist nursing care interventions that is responsive to the individual patients requirements.
- To be able to communicate highly sensitive information, ensuring patients and their families/carers are adequately supported. To be able to demonstrate through electronic and paper documentation and verbal communication the care undertaken and patient outcomes.
- To be responsible in leading on initial holistic assessment and reviews of care when required and discharge planning from service.
- To be able to communicate effectively to own team, extended team and outside agencies, ensuring continuity of care is maintained across the different care settings/home. To follow all agreed clinical procedures and statutory regulations related to Medicine Management.
- At identified shifts be responsible for dealing with any service enquiries, calls and referrals as required.

We provide palliative and end of life care support, including bereavement services, within Buckinghamshire and Hertfordshire and our patient services are available to adults, as well as patients transitioning into adult services (16 to 25 years.)
Bereavement support is offered for all ages to families of patients and those around them, as well as throughout the community, in schools and workplaces. We are proud to offer care in the place that best suits the individuals needs, whether that's at home, in a care home, in a short stay in our inpatient unit, out in the community, or in our purpose-built Living Well Centre facilities.
As a hospice care charity, we have offered this support, free of charge, for over 40 years.
