Job summary

As a Clinical Nurse Specialist, you will play a key role in coordinating and delivering specialist palliative and end of life care for babies, children and young people with complex needs. Working across hospital, hospice and community settings, you will ensure families receive seamless, holistic support in the place that is right for them, whether at home, in hospital or at Rainbows Hospice.

You will be a specialist resource for multidisciplinary teams, influencing palliative care practice, symptom management and service development across Derbyshire, while supporting education, training and quality improvement initiatives.

Main duties of the job

Provide specialist assessments, advice and support to babies, children, young people and their families with complex palliative care needs.

Coordinate care across hospital, hospice and community settings to ensure continuity and excellence of care.

Support children identified as being in the last year of life and facilitate Advance Care Planning.

Act as an advocate for children, young people and their families, ensuring their wishes remain central to decision-making.

Work collaboratively with healthcare professionals across multiple organisations and disciplines.

Lead and contribute to complex clinical and ethical decision-making.

Deliver education and training to healthcare professionals, students and partner organisations.

Contribute to service development, clinical governance and quality improvement initiatives.

Job description

Location: Derbyshire Healthcare United and Community with an expectation to attend Rainbows Hospice as required

Hours: 37.5 hours per week, including participation in a 7-day on-call rota

We're looking for an experienced and compassionate Clinical Nurse Specialist to join our Specialist Palliative Care Team, providing expert clinical leadership and specialist support across Derbyshire.
