Job summary
We are looking for a motivated and patient-focused Clinical Pharmacist to join our supportive and growing pharmacy team. Based at our Ernesettle Site in Plymouth, you will work alongside colleagues who are also based in Plymouth while providing clinical pharmacy support to a GP practice in Gateshead.Although the team works out of our Ernesettle Practice, we welcome applicants for on-site, hybrid, or fully remote working

Main duties of the job
You will provide clinical pharmacy support across a varied primary care workload, working closely with GPs, pharmacy colleagues and the wider multidisciplinary team.
The role will involve reviewing and optimising patients medicines, supporting safe repeat prescribing and medicines reconciliation, and helping to manage medication-related queries arising from hospital correspondence and day-to-day practice work. You will also provide clear, evidence-based advice to patients and colleagues, contribute to prescribing audits and safety initiatives, and support the management of long-term conditions.
You will be expected to use sound clinical judgement, communicate effectively and maintain accurate patient records. The successful candidate will be organised, collaborative and committed to patient-centred care, clinical governance and ongoing professional development.
Main duties:
- Conduct structured medication reviews with patients to optimise treatment and reduce risk.
- Support management of long-term conditions through evidence-based prescribing.
- Provide expert advice on medicines to clinicians and patients.
- Assist in medicines reconciliation post-hospital discharge.
- Contribute to prescribing audits, safety initiatives, and formulary adherence.
- Work closely with GPs and the wider clinical team to enhance prescribing quality.

Medication Management:
Medication Reviews Regularly review patient medications to optimise therapeutic outcomes and minimize risks of adverse effects. Adjust prescriptions as necessary in consultation with
the Senior Clinical Pharmacist and other healthcare providers.
Prescription Ensure prescription requests, acute and routine, are prescribed,
Drug Interactions and Contraindications: Vigilantly monitor for potential drug interactions and contraindications to prevent medication errors and ensure patient safety.
Discharge Resolutions: resolve medication issues and requests from hospital correspondence.
Pharmacy Tasks: complete the wide range of medicines-management pharmacy tasks on a daily basis.
Patient Care and Counselling
Patient Education: Provide comprehensive counselling to patients on medication use, side effects, interactions, and storage, enhancing patient knowledge and compliance.
Clinical Decision Support Assist healthcare providers in making informed clinical decisions regarding pharmacotherapy, utilising up-to-date clinical data and evidence-based guidelines.
Chronic Disease Management Participate in the management of chronic diseases by providing expertise in pharmacotherapy, contributing to patient care plans, and monitoring patient progress.
Collaboration and Communication
Multidisciplinary Team Collaboration: Work closely with a multidisciplinary team including doctors, nurses, and other healthcare professionals to ensure a coordinated approach to patient care.
Healthcare Team Support Provide drug information and pharmacological advice to healthcare team members, supporting the safe and effective delivery of treatments.
Patient Interaction Engage with patients to gather information on their health status, medication history, and treatment responses to inform care decisions.
