Job summary

An exciting opportunity has arisen for an enthusiastic and motivated Band 6 Nurse Specialist in Cardiology to join County Durham and Darlington Foundation Trust. The post is permanent for 30 hours per week and based at Darlington Memorial Hospital.

You will play a key role in supporting the management of patients with cardiology conditions including coronary heart disease, heart failure and arrhythmias throughout the trust. You will be an NMC registered nurse who is passionate about cardiology and has significant post registration experience, some of which must be in cardiology. An accredited clinical skills course and independent prescribing qualification are desirable or you will be capable of undertaking these once in post. You will be able to demonstrate excellent communication and interpersonal skills and be able to work independently whilst contributing effectively to the multi disciplinary team. Main duties of the job

  • To case manage patients with a cardiac diagnosis without direct supervision, reporting to the cardiovascular specialist nurses or wider multi-disciplinary team as appropriate.
  • To contribute to the development of service specific guidelines and protocols that are evidence based.
  • Assess patient need and develop a plan of care that is individualised and meets the needs of patients and carers.
  • Work closely with key members of the team from primary and secondary care to alter the management plan as appropriate. Communicate any document any changes in a timely manner.
  • Provide and support training for the multidisciplinary team in the management of CHD and heart failure.
  • To deliver cardiac and heart failure rehabilitation across the boundaries of County Durham and Darlington in line with agreed protocols.
  • To regularly attend and participate in training, peer support and clinical supervision.
  • To support service provision within the local area and wider foundation trust.
  • Assist patients to identify and manage risk factors using behavioural change to self-manage their condition.
